MyChart offers personalized and secure online access to portions of your medical records and enables you to securely use the internet to help manage and receive information about your health.

Getting started with MyChart

MyChart simplifies healthcare by centralizing your medical records, test results, and appointments in one secure platform. Whether you’re an inpatient or outpatient, it helps you schedule, manage, and participate in your care with ease.

Find your medication list, test results, upcoming appointments, preventative care information, allergies, immunizations, past admissions, insurance coverage and more – all in one secure place.

From your MyChart inbox, send a message to a member of your healthcare team and receive answers to non-urgent medical questions, as well as responses to your questions and requests about prescriptions and refills, test results, medical provider visits and referrals to other providers.

Use MyChart to book, cancel and reschedule some outpatient appointments at your convenience, as ordered by your physician, nurse practitioner or physician assistant. Add yourself to the waitlist to be notified if or when earlier appointment times become available.

Inpatients should talk with your care team directly and use MyChart to access portions of your medical record, including appointments, results, letters, and some notes that allow you to participate actively in your care.

MyChart helps serve as the control center for managing your health in a streamlined format. In it, you can view your to-do list, health reminders and manage proxy access (for parent, guardian, or other caregiver access if applicable) as well as access Medline Plus, a search engine for health topics. You can also share your medical data with other health professionals and request formal copies of your records.

How to activate MyChart

Activating MyChart is easy and requires an activation code provided by your healthcare team. Once registered, you’ll have access to a powerful tool to manage your health, communicate with providers, and view your medical history anytime, anywhere.

If you already use MyChart through Piedmont Healthcare, you will automatically have access to your Shepherd Center MyChart. You can visit the MyChart website and enter your existing Piedmont log-in credentials. If you already use MyChart through another Epic organization, you must create a Shepherd MyChart account. Your accounts can be linked if desired.

When you schedule your next appointment with your Shepherd provider, a MyChart activation code will automatically be emailed to you if we have your current email address. Also, at the conclusion of your next outpatient appointment, you will be provided with an activation code that will allow you to register for MyChart. If you wish to receive an activation code ahead of your next outpatient appointment, you may call Shepherd Center’s MyChart Support Team at 404-425-7250 or email [email protected] to request a MyChart activation code. Please provide the patient’s full legal name and date of birth in your request. After receiving this activation code, you will visit the MyChart website to set up your account and log in.

If you do not wish to have MyChart available during your admission, let staff know. Your case manager can assist you in setting up MyChart during your admission by providing an activation code and directions that will allow you to register. After receiving this activation code, you will visit the MyChart website to set up your account and log in. You may also contact Shepherd Center’s MyChart Support Team by calling 404-425-7250 or emailing [email protected] to request a MyChart activation code. Please provide the patient’s full legal name and date of birth in your request. The MyChart Support team can assist with the signup process.

Shepherd Center recommends that MyChart users enable two-factor authentication to protect the security of their personal health information in the patient portal. Download the MyChart two-factor authentication instructions.

Proxy forms

Complete a patient proxy form to allow authorized individuals to access a patient’s MyChart account, ensuring seamless communication and coordination of care. This feature is designed to support caregivers, parents, or guardians in managing a loved one’s medical information securely and efficiently.

Instructions for proxy forms

You must be 18 years or older to register for MyChart. If you are younger than 18 years old, you will need to have your parent or guardian register on your behalf by completing the appropriate proxy form, which you can download in PDF format below.

For various reasons, we also allow adult-to-adult healthcare proxies. You can download this form below, as well.

You should return the completed form to your Shepherd Center via one of these methods:

  1. Use the Shepherd Center Zix Secure Email Portal to securely email your attachment to [email protected]. Download the Zix Secure Email Account instructions for information on how to use Shepherd Center’s Secure Email Portal.
  2. Email using your email system to [email protected].
  3. Send via U.S. mail to: Health Information Management, Shepherd Center, 2020 Peachtree Road NW, Atlanta, GA 30309.
  4. Fax to 404-603-4520.
