Study details

This project convenes experts from Georgia Tech, Shepherd Center, and key stakeholders to address critical transition gaps.

Eligibility

  • Be a current patient or former Shepherd Center’s Pathways Day program or caregiver
    • Have graduated from inpatient or outpatient within the past 6 months, OR
    • Be a former patient of Shepherd Center’s Pathways Day Program (at least two-year post-discharge), OR
    • Be a caregiver of someone who has had a stroke
  • Prefer using English primarily for reading, writing, speaking
  • Be 18 years old or older
