Continuing your recovery journey

Your recovery doesn’t end when you leave the hospital. In fact, after discharge from inpatient rehabilitation, continuing outpatient therapy and specialized medical care is often needed to further improve cognitive function, physical abilities, and skills for daily living. This ongoing care helps you build on the progress you’ve made, while also preventing potential complications and promoting overall well-being. At Shepherd Center, we are committed to supporting you on your path to independence and a fulfilling life.

The brain injury residential program offers a structured, home-like environment to support your transition from inpatient care. Located in Decatur, Georgia, this program helps you practice functional living skills while preparing for a successful return to your home and community. With accessible features and 24-hour supervision, the six-bed residential facility provides a safe and supportive space for you to develop independence. Under the guidance of a multidisciplinary care team — including a physiatrist, nurses, therapists, and rehabilitation assistants — you’ll work on daily living tasks, behavior management, and medication management, all while building the skills needed for greater autonomy.

The brain injury day program at Shepherd Pathways is designed to help you transition from inpatient rehabilitation to greater independence. Held at our nearby facility in Decatur, Georgia, this program emphasizes community reintegration through activities like grocery shopping, accessing public transportation, and dining out. The structured, five-day-a-week schedule includes individual and group-based therapy, guided by a comprehensive team of specialists. This holistic approach ensures that you continue to progress toward your recovery goals while gaining the skills you need to thrive in your communities.

To build on the progress you’ve made in inpatient rehabilitation and keep advancing, our Beyond Therapy® program combines physical therapy with exercise physiology to target weaker muscles and nerve connections that may not have been fully addressed during the early stages of therapy. This program helps reduce complications, improve sensory and motor function, and equips you with the tools and knowledge to maintain physical readiness and support long-term recovery.
