How to request your records
- Download and complete the Authorization for Release of Information form (PDF).
- Fill out all necessary fields and sign.
- Submit:

Processing your request
- Timeframe: We process requests in the order received. Please let us know if you need the information by a specific date, and we’ll do our best to accommodate you.
- Fees: Georgia law allows for administrative and copying fees. For more information on fees, please call our Health Information Department at 404-350-7325. Fees are waived for copies sent directly to another healthcare provider for your ongoing care.
