Preparing for admissions

We strive to make the admissions process as seamless and stress-free as possible. To help ensure a smooth transition, we’ve prepared a comprehensive list of the necessary information and documentation required before you’re admitted into Shepherd Center.

Documentation and items needed before admission at Shepherd Center

  • Health insurance information: Please provide a copy of your major medical health insurance card. We will need your insurance company’s contact information, the insured person’s name, ID and group numbers, and employer’s name and phone number.
  • Medicare or medicaid card: A copy of your Medicare or Medicaid card.
  • Personal information: Your Social Security number and date of birth.

Additional documentation for specific situations

  • Provide a copy of your auto insurance card, including the policy number, your insurance agent’s name and phone number, and claim information.
  • If the auto insurance card and claim information are unavailable, please provide:
    • A copy of the accident report. If this report is unavailable, provide the following details:
      • Exact location of the accident (street intersection, city, state, county)
      • Date of accident
      • Police jurisdiction
      • Auto insurance med-pay letter
  • If you have legal representation, your attorney’s name and contact information

  • Location of the property
  • Address
  • Phone number and homeowner’s policy information
  • Name of the responsible party
  • If you have legal representation, your attorney’s name and contact information

  • Name of the property
  • Location and address
  • Property manager’s phone number
  • If you have legal representation, your attorney’s name and contact information

Important notice about ambulance transport costs

Shepherd Center assumes no responsibility for costs incurred for medical appointments outside of the hospital requiring ambulance transport or at discharge from the hospital.
