This session presents the outcomes of a high-risk cohort that was offered a transition support program (TSP). Those enrolled in TSP vs not enrolled experienced re-hospitalization at a rate of 5.7% vs 18% respectively. Recommendations for the clinical team are presented to prevent rehospitalization, improve health and safety outcomes, and promote self-advocacy. Opportunities to discuss outcomes and programmatic development will be presented in the panel discussion.
