Your journey with us begins with a comprehensive patient evaluation. From there, we’ll create a personalized care plan tailored to your needs. This plan may include ongoing medical care, physical, occupational, and speech therapy, wellness planning, mental health support, education, and more — all designed to support you.

How to request an appointment with the medical clinic, rehabilitation program, and wellness program

The Andrew C. Carlos MS Institute at Shepherd Center requires a referral for all new patients from a licensed healthcare provider. This allows us to streamline care for those in need of our services. The information below will help you learn how to request an appointment.

New patient appointments

The Andrew C. Carlos Multiple Sclerosis Institute accepts new patients with referrals from healthcare providers and specialists.

To learn how to begin the referral process, please call our case manager at 404-603-4962 from 8:00 a.m. to 4:00 p.m. ET, Monday through Friday.

Current and returning patient appointments

If you are currently receiving care at the Andrew C. Carlos Multiple Sclerosis Institute, you can request an appointment by calling 404-355-1144 from 8:00 a.m. to 4:00 p.m. ET, Monday through Friday.

New patient appointments

The Eula C. and Andrew C. Carlos Multiple Sclerosis Rehabilitation and Wellness Program accepts new patients with referrals from healthcare providers and specialists.

If you are currently receiving care at the Eula C. and Andrew C. Carlos Multiple Sclerosis Rehabilitation and Wellness Program, you can request an appointment by calling 404-603-4966 from 8:00 a.m. to 4:00 p.m. ET, Monday through Friday.

The Eula C. and Andrew C. Carlos Multiple Sclerosis Rehabilitation and Wellness Program offers a comprehensive wellness program open to all individuals and led by exercise physiologists.

Current patients in our medical clinic or rehabilitation program and those not currently receiving care at MS Institute at Shepherd Center can reach out to [email protected] to express interest in joining. A member of our Wellness team will be in touch with further details.

Appointment policies

It is very important that you attend all scheduled appointments. Availability in the MS Institute is limited, and a missed appointment can impact timely treatment for you and others.

While we understand that emergencies and other unexpected situations do arise, we request you provide us with a 24-hour notice in the event you are unable to make an appointment and need to reschedule.

Please arrive 30 minutes ahead of your appointment time.

If you arrive less than 15 minutes before your appointment time, we may need to reschedule your appointment. Your time with the provider is very important and arriving 15 minutes before your appointment time ensures that our nursing staff can prepare you for your visit and the providers are able to meet all your needs.

Preparing for your first appointment

We want to make the most of your visit here just as much as you do. Here are some tips on what to do before your first evaluation.

  • List questions and topics you’d like to discuss with your care team.
  • List of injuries you’ve had.
  • Document the symptoms you are experiencing and for how long.
  • Bring a care partner who can act as a second set of ears, take notes and prompt you if you forget anything.
  • Ask for clarity on anything you don’t understand. Medical terms can be daunting, and we want you to feel confident while you’re here and after you leave.

For your first evaluation appointment, please also bring with you the following:

  • Driver’s license or photo ID
  • Insurance card(s)
