Financial Assistance Plain Language Summary

Purpose

At Shepherd Center, we provide care to all patients who qualify for our programs, including those with limited financial resources. Patients can apply for financial assistance to help cover their financial responsibility for services provided. This applies to both hospital and physician services. However, certain high-cost drugs and consulting physicians are not included in the program.

Financial arrangements

Before receiving services, patients must make financial arrangements or provide insurance information. We accept insurance benefits as payment for verified, contracted plans.

For inpatient and day patients

With insurance

  • Insurance benefits are verified before admission.
  • Deductibles and non-covered amounts are due at admission. Any remaining balance will be billed later and is due within 30 days unless other payment arrangements are made.

Without insurance (self-pay)

  • Estimated charges are calculated before admission, and a Good Faith Estimate is provided.
  • A deposit covering 100% of the estimated amount is required. If needed, payment plans can be arranged on a case-by-case basis.
  • Self-pay patients receive a 50% discount on charges.

For outpatient services

  • If the patient has insurance, co-pays and deductibles are collected during service.
  • For self-pay patients, a Good Faith Estimate is provided, and financial assistance may be available after completing a financial screening.

Payment plans and financial assistance

  • Based on their financial situation, patients who cannot pay the full amount may be eligible for payment plans or charity care.
  • Short-term payment arrangements require a minimum payment of $50 per month. Payment plans can range from 3 to 36 months based on the total amount owed and approval from the Financial Services department.

Applying for financial assistance

Patients can apply for financial assistance by completing the Patient Financial Evaluation form (PDF). Eligibility is based on income, which must be less than 250% of the federal poverty guidelines. If the patient qualifies, the balance may be reduced or written off.

Exclusions

Certain drugs and treatments, such as Botox and Ocrevus, are not eligible for financial assistance. However, other related costs, like MRI scans or lab tests, may still qualify for charity care.

Extraordinary Collection Actions (ECAs)

Shepherd Center does not take extreme collection actions, such as reporting to credit agencies or garnishing wages, unless absolutely necessary. We follow a process that includes sending statements and allowing time for financial assistance applications to be submitted.
