How to Initiate the Enrollment Process
Step 1: Submit Your Inquiry
Begin your journey with Beyond Therapy and Wellness by submitting an enrollment inquiry through our webform. This allows our team to determine if our services align with your health, wellness, and recovery goals.
Step 2: Complete the New Client Questionnaire
Upon reviewing your inquiry, we will send you a detailed questionnaire to gather comprehensive information about your medical history and health objectives. This step includes a required medical examination or physical assessment by a qualified medical professional.
Step 3: Schedule and Attend Your Initial Client Evaluation
After receiving your completed questionnaire, we will contact you to schedule an initial evaluation with one of our experienced physical therapists. This thorough session includes assessments of your muscle strength, mobility, gait, balance, and other relevant outcome measures. We will then create a personalized treatment plan tailored to your unique needs.
